#Health2con @KBDesalvo Keynote (Acting Asst Secretary @HHSgov #PH3

Keynote: Karen DeSalvo

Karen Desalvo, the Acting Assistant Secretary for Health in the US Department of Health and Human Services, will speak on how the HHS is helping to build a better health care system by driving innovation, and how players across the health care system can come together to engage and empower consumers, and put patients at the center of their care.

Real time notes from the tenth annual Health2.0 fall conference.

Providing Affordable Quality Access to Care

Bring health to everyone.

Giving people the kind of coverage that many of us have taken for granted.
No Pre-Existing Conditions

Only about 9% of US Citizens now are lacking coverage.

Bending the cost curve. $550B saved against anticipated costs.

New Payment models are driving change in integrated delivery and better care.

Delivery System Reform:
1. Change provider payments (2018 – all payments in Value based payments)
0% in 2011, 30% in 2016

  1. Changing Care Delivery – putting patient at the center.
  2. Information Distribution. Data and tools.

We have to link data to payment model reform.

We are not where we want to be as a learning health system.

Making information more accessible.
Make it more understandable.

As we evolve the system we must pay more for outcomes.
Health and Public health must work together.

Public Health: What we do together as a society to assure the conditions in which everyone can be healthy.

Your zip code is a bigger predictor of health outcomes than anything else.
Build a stronger community Fabric.

#Health2con @boltyboy keynote and @Jonathan_Bush @epatientDave @DworkingDarren @GrandRoundsInc

Keynote: Matthew Holt

Matthew will introduce a new way to think about how health care delivery is changing because of technology. Somewhere in this Matthew and Jonathan Bush, CEO, athenahealth, will resume their long running banter!

Real time notes from the tenth annual Health2.0 fall conference.

Patients will change Healthcare

Being in Taxi cabs or Bookstores is not a great business these days.

Wake up Healthcare.

Three types of Patient

  1. Healthy
  2. Acutely Ill
  3. Chronically Ill

Healthcare is still one size fits all.
But we have the ability to target the individual.

Healthy – Asynchronous tools, Wellness tools, Telehealth

Ill – OMada Health and other tools

Chroically Ill
– Coaching
– Self Management
– Monitoring

Becoming Acutely Ill
– Diagnostics
– Data

Acutely Ill:
– Data Records
– System Tools
– Professional Tools

Diagnosis and Support:
work across all groups

Follow up tools

One size no longer fits all.

Why hasn’t Real Estate and Car buying changed.

Back with Jonathan Bush

“Health 2.0 needs to rebrand because the Government has got just about everyone on a shit EMR.”

“God bless Andy Slavitt”

“Remember there was a time when Allscripts was cool”

The world is done with digital medicine = they want networked medicine

EMRs will always suck unless they give you more information that you give it.

Future of Tech Enabled Services

@epatientDave Excellence and Convenience is important.

Experiencing the world of information Coordination across multiple providers.

“I want to deal with my Health Insurance company as much as my Car insurer.”

Owen Tripp – Grand Rounds, Inc.

67% of people were getting material mistakes in their treatment.
Could they scale a solution that gives everyone access to great diagnosis and expert opinion.

The Patient – Provider match is critically important
Same Day Second Opinion.

Patients can connect with Grand Rounds directly while undergoing treatment with another provider.

Cedars- Sinai Health System – Darren Dworkin

Leading hospital system in Los Angeles.

1,,000 bed hospital + 150 bed hospital added.
Added Ambulatory Services
Building Primary Care capacity.

“Building an Ecosystem”
Looking at how to partner.

“Technology is n’t part of our strategy.” Other larger goals. Technology is just an enabler.

Focusing on building platforms.
eg. Documentation system was not for digitization but a base for Clinical Decision Support.

The patient will become an extension of the care team. More diagnostics will take place in the home.

Jonathan Bush

Hospital Systems have maxed out. 6% of people are sick. 92% are not.

The opportunity for Health Systems is to go digital and then scale the services that ca be delivered digitally.
Emergency room may not scale.

Supporting 9,800 medical groups.

It is too exhausting to do traditional integration.

“I think the internet will be a big thing”

@SimpleTherapy

  • Helena Plater-Zyberk, CEO

Bright.md @bright_health

  • Julia Millard

Smart Exam System

integrates payment or billing to insurance.

HealthLoop – Todd Johnson CEO @Healthloop

Today- at discharge “Hope is the strategy”

Healthloop engages patients before surgery to help them prepare and follow sup afterwards.

Technology should get rid of the drudgery and support the clinicians.

#Health2con @paintmd , @edyson Forging new partnerships: Communities and Emerging Markets

Real time notes from the tenth annual Health2.0 fall conference.

Interviewer: Michael Painter @paintmd – Robert Woods Johnson Foundation

Esther Dyson @edyson – Way to Wellville

Way to Wellville, helping everyone to learn from experience in health and wellness and economic and environment development at the community level.

“We know what to do”

We need to give people the tools to manipulate their own lives and health.

Like a Personal Trainer. You have to do the work.
The community has to take ownership of the commitments and deliverables.

Getting buy-in is important. This is the role of community leaders. What works in their own community.

“We don’t want todo pilots. We want to roll out something that works”

Applicants have to have a multi-party involvement.

Share responsibilities instead of doing redundant work.

Health inequities

Huge issue.
Constantly learning.

Spartanburg, SC

working with Healthify

Unhealthiest town in the unhealthiest state in the USA.

Most of the issues that affect the community hapen outside the healthcare system

Healthify helps to automate many of the tasks for Case Managers.

Each person may need up to 10 referrals to different services.

Healthify helps to search for services and automate the applications.

  • Looking at referrals
  • Want to look at health and financial outcomes.

Spartanburg has a sizeable population that falls between Medicaid and Federal Services.

Spartanburg spends $300m on charitable and uncompensated care. as a result of ED visits.

Healthify can reduce referrals for case managers from 15 minutes to under 2 minutes.

Alameda County

Welkin Health

Serving East Bay underserved communities

#Health2con Ed Hernandez @dredhernandez , CA State Senator – Chair Committee on Health

Real time notes from the tenth annual Health2.0 fall conference.

CA has raised smoking age to 21.

Senator Hernandez pushed through the smoking age legislation.

Affordable Care

This is bigger than any other health legislation.

ACA has triggered lots of change:

  • Consolidation of Health Plans
  • Large Medical Groups
  • Hospital System mergers

Big challenges:
– 1/3 of CA population on MediCA

We wrote a law that provides an insurance card for patients but we didn’t address the network of providers needed.
– Patient need growing
– Doctors retiring

Demand in:
– Inner Cities
– Rural communities

The population with highest need but least access.

How do we control health care costs:

Proprietary platforms that don’t speak to each other.
Health records should belong to the patient.

We need to look at how we engage consumers in their own health.

The mobile device is available in even some of the most under resourced areas.

We have to learn to take advantage of the smartphone.

Generally acknowledged as being one of the funniest and most perceptive speakers in health care, Ian is an internationally known author, consultant and futurist. He was President at Institute for the Future in the 1990s, founded the Strategic Health Perspectives service, and is in more health care board rooms and conference halls that almost anyone. #Health2con Harvey Fineberg (Moor Foundation) Keynote address

Real time notes from the tenth annual Health2.0 fall conference.

Serious Solutions for Serious Illness

Keep patients in mind first.

Gordon and Betty Moore Foundation is a $6.4B Fund.

2/3 of people 65+ will have 2 or more chronic illness diagnosed.

What Americans want:

  • 71% want better quality of life over interventions.
Only 30% of care aligns with their preferences.
Last year of life accounts for 25% of Medicare spending.

Majority of people with chronic and late stage illness prefer to stay at home.
Only 1/3 of deaths occur at home.
30% are in ICU in the month preceding death.
25% of seniors visit an ED at least once per year.

People don’t want to be a burden but medical spending cripples families with bankruptcy.
25% of seniors are bankrupted by medical costs

Solving these challenges require a systems approach:

  • Public Education
  • Payment changes
  • Workforce development
  • Monitoring Systems

What to do:

  • Remember UX
  • Move beyond freestanding apps
  • Adapt to patient’s evolving needs
  • Connect families and clinicians

Seamless connectivity is needed.

“Put more life in to years and years in to life.”
