We need to give people the tools to manipulate their own lives and health.

Like a Personal Trainer. You have to do the work.
The community has to take ownership of the commitments and deliverables.

Getting buy-in is important. This is the role of community leaders. What works in their own community.

“We don’t want todo pilots. We want to roll out something that works”

Applicants have to have a multi-party involvement.

Share responsibilities instead of doing redundant work.

Unhealthiest town in the unhealthiest state in the USA.

Most of the issues that affect the community hapen outside the healthcare system

Healthify helps to automate many of the tasks for Case Managers.

Each person may need up to 10 referrals to different services.

Healthify helps to search for services and automate the applications.

  • Looking at referrals
  • Want to look at health and financial outcomes.

Spartanburg has a sizeable population that falls between Medicaid and Federal Services.

Spartanburg spends $300m on charitable and uncompensated care. as a result of ED visits.

Healthify can reduce referrals for case managers from 15 minutes to under 2 minutes.
