Social determinants have a tremendous impact on health, and understanding their impact on patients can help improve outcomes at all levels of healthcare. Learn how the State of Michigan is addressing this foundational issue, and results of the State’s combined Health and Human Services efforts.
Tim Becker kicked off Connecting Michigan with a visual demo. He rolled out a 64 page scroll. This is Michigan’s application for assistance.

Moving from Transaction and enrollment focused to Holistic, proactive and preventative approach focused on outcomes.
