Integrating human services with healthcare has become a valuable strategy to improve overall population health. Social determinants have a tremendous impact on health, and understanding their impact on patients can help improve outcomes at all levels of healthcare. Learn how the State of Michigan is addressing this foundational issue, and results of the State’s combined Health and Human Services efforts.

Tim Becker kicked off Connecting Michigan with a visual demo. He rolled out a 64 page scroll. This is Michigan’s application for assistance.

Michigan’s Department of Health and Human Services is working to improve the experience for the clients/citizens they serve.

MiHIN Mission: “Making data available at the point of care.”

Master Person Index from MiHIN is critical to helping reliably share information to reduce the situations where citizens need to re-enter frequently asked for data.

Healthy Michigan plan serves 600,000 citizens.

MiDHHS – Created Business Integration Center. Expanded to cover entire department.

Integrated Service Deliver is a journey to move from Program focus to Person-focus.

Moving from Transaction and enrollment focused to Holistic, proactive and preventative approach focused on outcomes.

Many applicants hire attorneys to get through the 64-page application process. Things should be simpler.
