You need a statewide service (Nationwide???) because patients wander.
Even if you reduce hospitalizations you don’t reduce the number of hospitals they appear at.

Built an eConsent Management system to avoid patients having to sign consent multiple times.
Direct integration with EMR to allow visibility to Lab Results and ADT Messages.
