Interoperability assumes we can structure all of the data using a standard vocabulary. However most data is in unstructured notes.

Structured and Unstructured data need to be combined. It drives completion of information and filling of data gaps.
Very few EMRs are cloud-based. Most are locked in closets or maybe data centers.
